Please arrive to your first appointment 15 minutes before your scheduled time in order to complete any paperwork required. To save time, you may print and complete forms ahead. These forms are located in the patient information section of our website.

Please wear loose fitting, comfortable clothing to your appointment, such as athletic wear. Your therapist will need to be able to evaluate your injury. This means they will need to see, touch and move your affected body area. Sturdy, comfortable shoes should also be worn to allow you to participate fully and safely in your evaluation.

We need you to bring the following:
1. Photo Identification (valid driver’s license, valid state ID, school ID, or valid passport)
2. Doctor referral/prescription for physical therapy (if needed by your insurance plan)
3. Insurance card(s)
4. List of all of your medications, vitamins, supplements and what you take them for
5. Any MRI/CT/XRAY reports
6. Your medical history – this should include any prior surgeries
7. Your calendar so we can set up future appointments that are convenient with your schedule

Foremost, you should expect to be evaluated by a Licensed Physical Therapist and be treated by a Licensed Physical Therapist or a Licensed Physical Therapist’s Assistant. You should expect for your treatment to include all or some of the below activities (condition dependent):
1. Education about your condition, injury
2. Stretching to reduce stress on joints and improve flexibility
3. Manual therapy to improve or maintain range of motion
4. Exercise to strengthen muscles and to improve coordination, balance and endurance
5. Postural reeducation
6. Home Exercise Program and instruction
7. Injury prevention
8. Pain management
9. Modalities such as ultrasound, Electrical Stimulation, Traction, heat and/or cold therapy to relieve pain

Your first visit will last one hour to one hour and fifteen minutes and will include a comprehensive evaluation by a Licensed Physical Therapist. This evaluation includes a detailed review of your medical history, prior level of function, current level of function, cause of injury, and review of diagnostic tests.
You will be asked to complete a functional ability survey and communicate your goals for physical therapy related to your functional ability. Advanced PT has partnered with Focus on Therapeutic Outcomes Inc (FOTO®), a database for rehabilitation outcomes to measure and compare our patients’ results with national data from thousands of clinics and millions of patients across the United States. We will explain your survey to you and use this as another tool to formulate your treatment plan.
By the end of your evaluation, you can expect your physical therapist to have communicated the evaluation findings and developed a treatment plan with your input to assist you in meeting your specific goals. Finally, after your evaluation, you will also begin to receive targeted treatment based on your evaluation findings. Your therapist will provide you with a home exercise program (HEP) to complete between therapy visits to allow you to be an active participant in your recovery.

During the assessment, a licensed healthcare professional will take a detailed subjective report of your musculoskeletal complaints, as well as assess your movement impairments and discuss how this is affecting your activities of daily living. In addition, an assessment on joint integrity, range of motion, strength and flexibility could be performed. This will allow us to help guide your medical options and recommend the next best steps. This evaluation is typically 10 minutes.

After your first appointment, your physical therapist will discuss a treatment plan for your care including estimated frequency and length needed for your specific issue. The recommended number of visits can range from a few visits to several months depending on your injury and the functional goals that you want to accomplish.

Most insurance plans allow an individual to be seen by a physical therapist via Direct Access. If you contact our office, we can verify your insurance and let you know if any additional information is needed prior to your appointment.

Yes. We treat patients with work-related injuries and can assist with coordination of care with your worker’s compensation carrier.

A deductible is the dollar amount that must be paid by the insured (ie, patient) before their insurance policy will start paying for the covered expenses. A patient is responsible for allowable charges until the deductible is met.
A co-payment (co-pay) or co-insurance is the dollar amount the insured (ie, patient) is responsible for paying for each visit. This dollar amount may be noted on the front of your insurance card. Advanced PT will collect your co-payment at the time of your service. Once your maximum out of pocket for the year has been met, you are no longer responsible for your co-payment or co-insurance.
Maximum out of pocket is the most you to pay for covered services in a year. This includes the amount spent on deductibles, co-payments, and charges from covered medical services. Once this amount has occurred, your insurance pays 100% of the cost of the covered benefits.
